Supported Family Living / Independent Living

Affinity Community Care supports individuals in developing daily independent living skills within their own homes. Our services include personalized assistance with activities of daily living, health maintenance, social and leisure skill development, and guided community involvement to promote overall well-being and independence.
Behavioral In Home Habilitation
Affinity Community Care (ACC) provides specialized habilitative services for individuals with developmental disabilities who also experience chronic or severe mental health conditions. Our program is designed to help participants acquire, retain, and enhance self-help, social, and adaptive skills, supporting their ability to live more independently and engage meaningfully in their community.
Shared Living

Affinity Community Care offers daily instruction in independent living skills within a contracted provider’s home. Services include support with daily activities, health maintenance, social and leisure development, consistent supervision, and structured recreational opportunities in the community to promote stability and personal growth.
ACC also provides Community Integration, Respite and Transportation
